Dynamic lymph node mapping using a near infrared fluorescent dye

Cancer metastases is a leading cause of cancer related deaths. The lymph system is a channel for transporting tumor cells and therefore plays an important role in pathogenesis of breast cancer metastases. Thus, sentinel lymph node mapping followed by biopsy has become a standard for diagnosing disease stages. In this presentation, we demonstrate a rapid, non-invasive dynamic fluorescent optical imaging technique for visualization of lymph nodes, lymph channels, and pulsating lymph flow using a non-specific NIR dye in a preclinical model of swine as an initial step toward utilizing molecularly targeted optical agents. Intra-dermal injections of indocyanine green (ICG) solutions (3-322 µM) were administered in the lymphatics of Yorkshire swine using a novel microcanula/catheter device. Dynamic optical imaging was performed by exposing the swine skin to a 0.25-mW/cm2, 785-nm plane wave and the re-emitted fluorescence at 830 nm was detected using an intensified charged-coupled device (ICCD) camera outfitted with band-pass and band rejection filters. From the dynamic images of the lymphatic trafficking of ICG we (1) mapped the lymphatic network of axillary, mammary, and inguinal regions of swine (2) characterized the dynamics of propelled lymph flow. Our results demonstrate the feasibility of the use of the optical imaging method for sentinel lymph node mapping and thus support the use of molecularly targeted contrast agents for future molecular imaging to track cancer micro-metastases.
