Osteosclerosis in idiopathic myelofibrosis (IMF) is likely related to overproduction of osteoproteingerin (OPG). 21 patients with IMF were studied . Plasma OPG levels were assayed by OPG ELISA and compared with normal volunteer controls. The results showed that the plasma OPG levels were 4035 + 897 (95% CI, 2163-5907 pg/ml), and 1174 + 83 (95% CI. 992-1357 pg/ml) (p=0.01) in patients with IMF and controls respectively. Plasma OPG levels also were correlated with degree of osteosclerosis by reading the bone window in the CT scan films and a positive correlation ( r2=0.68, p=0.04) was found. No correlation of plasma OPG levels and WBC counts, hemoglobulin levels, platelet counts, duration of disease, degree of marrow fibrosis, and degree of splenomegaly was found. These results suggest that osteosclerosis in IMF is likely related to overproduction of OPG. The overproduction of OPG may be related to the excessive production of TGF-ß or other growth factors.
